Early Childhood Partnership of Adams County

The C4 will address social determinants of health in a low-income, high need Colorado county. It is a place-based integrated neighborhood wellness center that will provide easy access to high-quality, coordinated whole-person healthcare and social services. Funding will support a combination of direct services and “backbone” activities essential to ensuring the sustainability of the C4.
