Grant Summary: Grand County’s patient navigator program helps uninsured, low-income, and underserved patients navigate through the healthcare system while reducing barriers to care. The Grand County Rural Health Network, working in collaboration with the Kremmling Memorial Hospital District’s (KMHD) two primary care clinics, propose to continue the current nurse and lay navigation program that removes barriers to care and provides care coordination. We anticipate increasing the coordination of healthcare services, improving chronic disease management, and improving patient engagement through the continued use of the Patient Activation Measure (PAM). We will also be able to synthesize our data across funding streams to allow us to better track insurance codes and create longterm sustainability for the program.
