Grant Summary:

Promoting Health: Improving Patient Outcomes for Childhood Obesity through a Coordinated System of Care (“Promoting Health”) is a unique pilot clinical/community integration project aimed at vulnerable populations in Chicago that will establish a model for a coordinated system of care for pediatric obesity. The innovative use of technology plus a dedicated care coordinator will help ensure that patients and their families are able to successfully receive the appropriate care and improve their health outcomes. Promoting Health also seeks to improve the quality of care for prevention and management of pediatric overweight/obesity in clinics through continuous quality improvement, obesity-related education, and implementation of changes in practice.
