Grant Summary

This funding will provide resources to the Department of Medical Informatics (MI) at OUSCM to engage key stakeholders within the Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS) and recipients of Oklahoma’s 2703 Medicaid Health Home Contracts in technical assistance targeted towards Health Home implementation. The technical assistance will encompass workflow re-design, clinical task-shifting, outcomes reporting, Electronic Medical Record (EMR)/Health Information Exchange (HIE) readiness, and population registry management centered on an adaptation of an evidence-based Collaborative Care (CC) model of Behavioral Health and Primary Care Integration.
